The short answer is that they work in a similar fashion. They were originally developed to treat pulmonary arterial hypertension. During clinical trials, the test subjects (old men suffering from PAH) started sporting wood like a bunch of zit-faced teens. The primary mechanism of both is muscle relaxation resulting in increased blood flow to the soft tissue (your pecker is made up of this soft tissue. The pharma companies quickly discovered that there are more guys interested in supplementing their peckers (whether they need it or not) than there are actual sufferers of PAH.

I think that the main difference between the two is the time to effectiveness and the duration of effectiveness. Cialis works faster (15 mins) and last longer (up to 36 hours) whereas viagra kicks in after 30 mins and lasts for 5+ hours. Cialis also seems to be about 4-5X more expensive.

Common side effects include heartburn and and the smell of melting latex in your hotel room.
